Administering Medicine

We will only administer medicines if prescribed by a doctor/nurse or approved over-the-counter (OTC) medicine*.  Medicine is only administered with the consent of the parents or carers and with a completed and signed form.  

* OTC medicine will not be administered to children under 6 years of age.   

Approved OTC medicines: Paediatric Paracetamol and Children’s antihistamine

We also need information about your child if they have an asthma inhaler. Please make sure any inhalers are in date. 

Absence Requests

Although we are unable to authorise leave for a family holiday, we can consider authorising an absence for exceptional circumstances and ask that you complete this form if you wish to take your child out of school.
